A Research on the Reimbursement of Public Hospital

ObjectivesThe objective of this research is to review and describe the financial situation of public hospital for recent years,to evaluate the effects of health reimbursement policy, and to find out the prboblems of reimbursement of public hospital.Finally,to make some suggestion for the improvement of health reimbursement policy of public hospital.MethodsThis research has analyzed a situation of the economic operation of public hospitals in three provinces in 2001,2004 and 2006.What is more,six typical hospitals of these provinces are chosen as case study,the situation of which in 2007 is also researched as an augment.ResultsThe result has indicated that drug incoming is shrinking into 45-50%of the total revenue as a mayor way of imbursement of public hospital from 2001 to 2006.The proportion of drug incoming is too much higher for whether outpatient or inpatient.General speaking,the proportion of drug incoming is 46-65%for outpatient, and inpatient is 40%.In the case study,this proportion of outpatient is up to 51-80%, and inpatient is 24-50%in the six hospitals.On the other hand,some items related to the incoming proportion of human resource which indicates the value of the doctors and nurses are too low,for instance,the proportion of diagnose and register incoming is only 2%,and surgery incoming is 3-5%.What is noticed,the proportion of laboratory and therapy is increased on different level.As a result of the national drug policy in 2006,the deduct percentage is decreased but still higher than 15%.In the six typical hospitals,the index is almost or a little lower than 15%,but some are still higher than 20%.If deducted the drug incoming,and all the pre-condition is not changed,to be rebalanced, the public hospitals of three provinces should be compensated for 880,290 and 320 million yuan respectively by financing,or the pricing of all the medical item raised 15%,9%and 5%respectively.ConclusionsThe research has suggested that public hospital could be reimbursed through enhancing finance subsidy or raising price of medical items if drug incoming were cancelled step by step.However,to set the imbursement of public hospital right,a reformation of system and mechanism is necessary.On one hand,the payment of hospital should be advanced into higher and more multiplex form.On the other hand,to accomplish clinical criterion and strengthen the surveillance of clinical quality and physician's behavior should also be engaged.
