Research on Influencing Facts of Single Diseases Hospitalization Expense in Polyclinic

ObjectiveThe excessive growth of medical expenses has placed a heavy financial burden to the country,units and individuals.It also constraints the development of the health cause.How to solve the issue of "being unaffordable and difficult to see a doctor" has become the key point and difficulty in the course of our medical care reform.It is necessary to research one which has regional characteristics to control medical expenses combining the domestic and foreign disease administrative patterns and theories,according to the actual situation of the hospital.It can give references for health administrative sectors about increasing the hospitals economic and social benefits,enhancing the management of medical service quality and formulating unified expenditure standards of diseases by studying the components and influence factors of hospitalization expenditure of diseases in some hospital of China Medical University and discussing scientific and standardized administrative means to restrict the enhancement of medical expenditure.Materials and methodsThe basic information on the home pages of medical records of 11059 inpatients was collected with using the review investigation method during from 2005 to 2007 in hospital.The information of 5 common and frequently-occurring disease was selected based on the international classification of disease(ICD-10).Several targets including patient's social economic characteristics,disease characteristics,diagnosing and treating quality and data of each part which constitutes the total hospitalization expenditures were investigated on basis of the basic information on home pages of medical records.All the information was used to establish the database and proofread in Excel,and the single factor analyze,the multi-groups variance analysis and Multiple stepwise regression which is used to establish equation are carried on using SPSS for the windows 13.0 software.ResultsDuring the constitution of hospitalization expenditure of 5 single diseases in hospital,the proportion of charges for medicine is bigger,According to the result of analysis,the average hospitalization expenditure of different diseases were different among the different ages,the sexes,days of hospitalization,the payment way,the diagnosing and treating quality and so on,in which the influence of the days of hospitalization、Secondary diagnosis and the payment way were bigger.ConclusionsThe days of hospitalization、the payment way and charges for medicine are the major to increase medical expenditure.Therefore,while reducing the medical expenditure,we should put effort on controlling the proportion of charges for medicine, reducing the days of hospitalization by Clinical Pathway,establishing scientific diagnosing and treating standards and consummating medical insurance system, strengthening management and guide the reasonable medical consumption.
